• Good Scholastic Record

a. For Doctoral Program, the average rating in the master’s level must be at least 90% (1.50) or better.
b. For a Master’s Program, the average rating in the bachelor’s degree must be at least 85% (2.0) or better.

• Original copy of Certificate of Honorable Dismissal or Certificate of Transfer Credential
• Photocopy of Special Order – School File (if applicable)
• Informative Copy of Transcript of Records (for evaluation purposes)
