If one has attended college after graduation from high school, then he / she is considered Transfer applicant. A transferee is a student accepted by the department after undergoing necessary procedures prior to enrolment.
:: Requirements:
• A letter of application addressed to the Admissions Officer
• Photocopy of Transcript of Records(TOR) from college(s) attended before (for evaluation purposes)
b.1. Has an average of at least 80% in courses/subjects taken in other school.
b.2 No failure in any subject related to course/program applied for.
c. Pre-exam interview by the Department Dean
d. Passed Transferee's Examination given by the College.
e. Transfer Credentials (Honorable Dismissal)
f. Certificate of Good Moral Character
g. Clear photocopy of birth certificate that is duly authenticated by the Ntional Statistics Office (NSO)
h. Good physical health attested by results of general physical health examination and laboratory examinations which includes CBS, Fecalysis, Urinalysis, and Chest X-ray. Nursing, Physical Therapy, and Medical Technology applicants must also submit Hepatitis B screening results.
i. Police/NARCOM Clearance
j. I.D. photo 2 x 2 - 4 copies, 1 x 1 - 4 copies (with red background)
k. Long ordinary mailing envelope.
